据2006~2007年度卫生部全国细菌耐药监测结果显示,全国医院抗菌药物年使用率高达74%。而世界上没有哪个国家如此大规模地使用抗生素,在美英等发达国家,医院的抗生素使用率仅为22%~25%。中国的妇产科长期以来都是抗生素滥用的重灾区,上海市长宁区中心医院妇产科多年的统计显示,青霉素的耐药性几乎达到100%。而中国的住院患者中,抗生素的使用率则高达70%,其中外科患者几乎人人都用抗生素,比例高达97%。
另据1995~2007年疾病分类调查,中国感染性疾病占全部疾病总发病数的49%,其中细菌感染性占全部疾病的18%~21%,也就是说80%以上属于滥用抗生素,每年有8万人因此死亡。这些数字使中国成为世界上滥用抗生素问题最严重的国家。 2011年4月7日是世界卫生日,首届合理用药大会在这一天召开,恰好契合了世界卫生日的主题——“抵御耐药性:今天不采取行动,明天就无药可用”。从相关部门获悉,北京共有协和医院、积水潭医院、宣武医院、人民医院、北医三院等首批20家三级和二级医院正在参与处方集中点评。重点监测内容就是抗生素的使用情况。 市卫生局成立了北京市医疗机构药事管理专家委员会,委员会下面分为8个工作组,包括处方点评组、药事管理组、抗菌药物与细菌耐药监测组、合理用药与药品不良反应监测组、中药组、药品质量管理组、临床药学组、学科建设与药学研究组。
其中,处方点评组的专家委员将每月收集二三级医院和社区卫生服务机构的处方点评数据。二三级医院每个月要随机抽样100张处方,进行数据分析,包括注射剂占门诊处方的比例,抗生素占门诊处方的比例,每张处方平均用药数量,每张处方平均金额等。这些数据需要上报专家委员会。
此次处方集中点评的重点监测对象是抗生素,尤其是卫生部限制使用的特殊管理类抗生素,如万古霉素,按照国家规定需要由高级职称的医师开具处方,必要时还需要院内会诊,因此在处方集中点评时将会备受专家关注。
原则上,三级医院的门诊处方中,抗生素用量一般占整张处方用药量的15%左右。如果某些医疗机构对抗生素的用量大大超出平均水平,将被公示和干预。此外,专家委员会还将定期对全市的各种抗生素用药进行大排名,辅助治疗的抗生素用药或中药注射剂如果位列前十名,即说明医疗机构存在不合理用药问题,将被干预和调整。
为了扭转滥用抗菌素的现状,世卫组织推出了6项政策一揽子计划,包括与社会多方合作并制定国家计划、加强监测和实验室能力、确保有质量保证的基本药物的连续可及、减少抗菌素在食用动物中的使用、强化感染预防与控制、支持创新和研发新工具等。
与此同时,卫生部也把抗菌素药品的使用作为考核医院政绩和工作的重要指标,并将组织全国性的督导行为,对于违纪违规的医院、医生采取严厉措施。如果有些医疗机构不执行卫生部相关规定,可能被降级。
规范使用
2011年4月卫生部发布了以规范抗菌药物临床应用、限制不合理使用、制止滥用的《2011年全国抗菌药物临床应用专项整治活动方案》,后又发布了《抗菌药物临床应用管理办法》征求意见稿。
这一系列对抗生素的限用令正在逐步产生市场传导效应,必将对医院抗生素制剂需求造成明显抑制。业内人士普遍预计,限制级和特殊级抗生素使用量可能下降30%~50%,销售额下降20%,其中,临床滥用较多、替代性较强的三代和四代头孢类、抗耐药复方制剂和喹诺酮类抗菌药物所受的影响将是最大的。向左转|向右转
细菌产生耐药性主要通过以下三个途径:
1.产生两种灭活酶。一是水解酶,可水解青霉素、头孢菌素;二是合成酶,可催化某些基团结合到氨基甙类等抗生素上,使之失活。
2.细菌的细胞膜通透性改变。细菌可通过各种途径使抗生素不易进入菌体,如某些杆菌的细胞外膜对青霉素等有天然屏障作用;还有些细菌可通过细胞壁水孔或细胞外膜通道的改变,使抗生素不易渗透至细菌体内,产生耐药。
3.细菌体内靶位结构改变,使抗生素不易与之结合。
适应症下会标注“本品适用于敏感菌所致......”字样;
药理作用下会标注“本品属......类抗菌素”字样。
在生活中,不少人认为抗生素是退烧药而随便使用,这是一种误区。其实不是所有的发热都是由细菌感染引起的,常见的伤风感冒通常由病毒所致,也发热,用抗生素毫无用处。而服用一般解热镇痛药大多可以奏效。患了病毒性感冒后,应到医院诊疗,只要加强护理,适当休息,多喝开水,给予易消化的饮食,通常会很快恢复健康。
俗话说,“是药三分毒”。滥用抗生素危害就更大了。首先,它会诱发细菌耐药。病原微生物为逃避药物,在不断地变异,耐药菌株也随之产生。目前,几乎没有一种抗菌药不存在耐药现象;其次,它会损害人体器官。抗菌药在杀菌的同时,也会造成人体损害,如影响肝、肾脏功能、胃肠道反应及引起再生障碍性贫血等;再次,它还会导致二重感染。在正常情况下,人体的口腔、呼吸道、肠道都有细菌寄生,寄殖菌群在互相拮抗下维持着平衡状态。如果长期使用抗菌药,敏感菌会被杀灭,而不敏感菌乘机繁殖,未被抑制的细菌、真菌及外来菌也可乘虚而入,诱发又一次感染。
是否使用抗生素治疗,首先应去医院做个血常规化验,查一下白细胞情况,搞清感冒的性质。如果是病毒性感冒,使用抗生素毫无用处,应使用抗病毒药物;如果感冒症状是由细菌感染造成的,则需使用抗生素。病人可根据感冒的症状进行自我初步判断:体温不高,痰是白色的,鼻水是清的,一般可初步判断为病毒性感冒;痰是黄色的,则可能是细菌感染。
小孩感冒应先到医院做血常规检查,确诊是病毒感冒还是细菌感染。如病毒感冒不需用抗生素治疗,如果是细菌感染,应在医生指导下选择适合儿童用的相对安全的抗生素治疗,如:青霉素类、头孢类抗生素。需要特别提醒的是,氨基糖苷类抗生素一定要慎用,因为它有耳毒性,有造成儿童耳聋的风险。
阅读抗生素的药物说明书时应重点关注适应证、用法、用量等细节。小儿用药剂量和大人不同,而有的家长不按说明服药,拿成人用的药量给孩子服用,以为只要对症就能治病。殊不知,有些抗生素对骨骼发育会产生抑制作用,成人可用,但孩子万万吃不得。需要特别提醒的是,儿童颗粒剂抗生素要用温开水调服,因为抗生素的稳定性不高,水温太热会影响药效。同时要避免用牛奶、饮料等服药。另外,患者在服用头孢类抗生素期间及治疗结束后72小时内(三天)应避免摄入含酒精的饮料,否则会引起戒酒硫样反应,人会有醉酒的感觉。还要避免吃奶酪,因为奶酪中有酪氨酸,会与药物发生反应。
