Interleukin23(IL-23)isaheterodimericcytokinecomposedoftwodisulfide-linkedsubunits,ap19subunitthatisuniquetoIL-23,andap40subunitthatissharedwithIL-12.Thep19subunithashomologytothep35subunitofIL-12,aswellastoothersinglechaincytokinessuchasIL-6andIL-11.Thep40subunitishomologoustotheextracellulardomainsofthehematopoieticcytokinereceptors.Theratp19CDNAencodesa196aminoacid(aa)residueprecursorproteinwithaputative19aasignalpeptideand177aamatureprotein.Thematureratp19proteinshares88%,78%,76%,75%,71%,and70%aasequenceidentitywithmouse,human,canine,equine,guineapig,andbovine,respectively.Activatedmacrophagesanddendriticcellsexpressp19andp40concurrentlytoproduceIL-23.ThefunctionalIL-23receptorcomplexconsistsoftworeceptorsubunits,theIL-12receptorbeta1subunit(IL-12Rbeta1)andtheIL-23-specificreceptorsubunit(IL-23R).IL-23andIL-12haveoverlappingbutdistinctBIOLOGicalactivities.IL-12drivesdevelopmentofTh1cellsandinducesproductionofIFN-gammabyNKcells;IL-23inducesproliferationofTh17cellsandCD4+memoryTcellsdistinctfromTh1whichproduceIL-17,apotentproinflammatorycytokine.IL-23alsodrivesIL-17productionbyNKcellsandneutrophils.WhilebothIL-12andIL-23pathwaysrespondtoinfectiousagents,theIL-23-IL-17immunepathwayinducestheearliestrecruitmentofneutrophilstothesiteofinfectionwhilethemoreclassichostdefenseandcytotoxicresponseisstimulatedbyIL-12.DysregulationoftheIL-23-IL-17immunepathwayhasakeyroleinorgan-specificautoimmuneinflammatorytissuedestruction.
