1.INTRODUCTION
Thyroidperoxidase(TPO)presentattheapicalplasmamembraneofhumanthyroidcellscatalyzesthefirststepinthyroidhormonesynthesis.TPOoxidizesiodide-anionsandiodinatessometyrosylresiduesofthyreoglobulin(TG),leADIngtotheformationofmonoiodotyrosine(MIT)anddiiodotyrosine(DIT).CouplingofMITandDITyieldstriiodotyronine(T3)andtetra-iodotyronine(T4).TPO(previouslyknownasthyroidmicrosomalantigen)isafrequentepitopeofautoantibodiesinautoimmunethyroiddisease.TPOautoantibodies(TPOAb)aredetectableinover90%ofpatientswithHashimoto"sthyroiditisandover70%ofthosewithGraves"disease(autoimmunehyperthyroidism),irrespectiveofthefunctionalstateofthethyroidgland.Postpartumthyroiditisisaspecialformofautoimmunethyroiditisobservedfollowingpregnancyandmayinvolvetemporaryhyperthyroidismorhypothyroidism.Itaffectsabout5%ofallwomenwithinayearaftergivingbirth.Therisksarehigherforwomenwithautoimmunedisorders(suchastype1diabetes),positiveanti-thyroidantibodiesorahistoryofpreviousthyroiddysfunction.ModeratelyelevatedTPOAbvaluesaresometimesfoundinpatientswithoutotherevidenceofthyroiddisease,especiallyintheelderly.TheclinicalsignificanceoflowlevelsofTPOAbineuthyroidsubjectsisstillunknown.Whethertheseindividualsshouldbeconsiderednormalremainsinquestionuntillong-termfollow-upstudiesshowthattheydonothaveanincreasedriskfordevelopingthyroiddysfunction.

TheGenWayanti-TPOELISAisintendedforthequantitativedeterminationofautoantibodiestoTPO(thyroidperoxidase)inhumanserumorplasma(citrate).
3.PRINCIPLEOFTHEASSAY
ThequantitativeimmunoenzymaticdeterminationofautoantibodiestoTPOisbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique.Microtiterstripsareprecoatedwithrecombinantthyroidperoxidase(TPO)tobindcorrespondingantibodiesofthespecimen.Afterwashingthewellstoremoveallunboundsamplematerialhorseradishperoxidase(HRP)labelledanti-humanIgGconjugateisadded.ThisconjugatebindstothecapturedTPO-specificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct.TheintensityofthisproductisproportionaltotheamountofTPOspecificIgGantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader.
