| Introduction | Q-Feverisadiseasethatresultsfrominfectionwithsmall,polymorphandgram-negativebacteriacalledCoxiellaburnetii.AfteranoutbreakinBrisbane,Australia,theresponsIBLeorganismwasisolatedandnamedCoxiellaburnetiiinhonourofDr.HeraldRaeCoxandSirFrankBurnet.NewmolecularresearchdemonstratedacloserelationshiptoLegionella.ThezoonosisQ-FeverisfoundeverywhereexceptNewZealand(nodataavailable).Thereisanextensivereservoir(mainlyticks)ofC.burnetii.Ticksareanimportantvectorofthepathogeninthetransmissionbetweendomesticandwildlifeanimals.Buttheticksareunimportantinthedirectinfectionofhumans.Cattles,sheepandgoatsareusuallythesourceoftransmissionofthismicroorganismtohumans.Howevercats,dogsandrabbitsarealsoimportantinthisregard.InmostinstanceshumansbecomeinfectedwithCoxiellaburnetiifollowinginhalationofcontaminatedaerosols(respiratorytract).TheincubationperiodforQ-Feverinhumansisabout2weeks.Theresultingillnesscanbedividedintoacuteandchronicvarieties.Duringtheacutephaseofillnessantibodiestothephase2-antigenareformed.Antiphase-1antibodiesinhightitersaretypicalforachronicdisease. InareaswhereQ-Feverisendemic,12%ormoreofthepopulationhaveantibodiestoC.burnetii.Mostoftheinfectionsaresubclinicalorundiagnosed. Theacuteinfectionshowssymptomsofhighfever,shivers,musclepainandheadache.Lateronmoreseverediseasessuchaspneumoniaorhepatitiscanoccur.Infectionsduringpregnancycanleadtoanabortorprematurebirth.Approximately1%ofallinfectionsbecomechronic.ThemostfrequentorganmanifestationinQ-Feverisendocarditis.Diagnosticmethods Complementbindingreactionisstillused IFT(immunofixationtest) ELISA Cellculture PCR IFTaswellasELISAdifferentiatebetweendifferentantibodyclasses(IgG/IgM/IgA).AcutephaseofQ-Fever IgMspecifictophase2after2-3weeks IgGapproximately2monthsafterinfection.ChronicphaseofQ-Fever From6weeksupto4monthsafterinfectionphase1IgGandIgAantibodiescanbedetected. |
