IntendedUse:TheTick-borneencephalitisvirusIgM-ELISAisintendedforthequalitativedeterminationofIgMclassantibodiesagainstTBEvirusinhumanserumorplasma(citrate).
Introduction:Tick-borneencephalitis(TBE)virusisaflavivirusofthefamilyTogaviridae.Itisanenvelopedsingle-strandedRNAviruswithcubicicosahedralsymmetryandrangesinsizefrom20-80nmindiameter.OntheEuropeancontinentonlytwoantigenicsubtypesexistwhichshowonlylittledifferencesintheirstructuralproteins.TBEvirusismainlytransmittedbyticks.Thedegreeofcontaminationofticks(andthushumans)incentralEuropeincreasesfromwesttoeast,andanybodymaybeaffected.Specificantibodydevelopmentyieldsalife-longimmunity.TBEisthemostimportanttick-transmitteddiseaseofman-besideLymedisease,whichiscausedbythespiRocheteBorreliaburgdorferi.Theclinicalcourseofthediseasedependsontheimmunestatusoftheinfectedpersons.Ahighvirusproductionintheprimaryinfectedtissuesisrequiredforthepassageoftheblood-brainbarrierandtheresultingseveremanifestationsinthecentralnervoussystem.
Ofthetwosubtypes,RSSEcausesthemoresevereinfection,havinganincidenceofmortalityofupto25%insomeoutbreaks,whereasmortalityinCEEseldomexceeds5%.AninactivatedTBEvirusvaccineisavailableinEuropeandRussia.Thepresenceofvirusresp.infectionmaybeidentifiedby:

Principlesoftheassay:ThequalitativeimmunoenzymaticdeterminationofIgM-classantibodiesagainstTBE-VirusisbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique.
MicrotiterstripwellsareprecoatedwithTBE-Virusantigenstobindcorrespondingantibodiesofthespecimen.AfterwashingthewellstoremoveallunboundsamplematerialhorserADIshperoxidase(HRP)labelledanti-humanIgMconjugateisadded.ThisconjugatebindstothecapturedTBE-Virusspecificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct.TheintensityofthisproductisproportionaltotheamountofTBE-VirusspecificIgMantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader.

LimitationsoftheTest:Crossreactionsofantibodiestootherflaviviridae(DengueVirus,YellowFeverVirus,WestNilVirus)mayoccur.
Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasSEROlogicaldata.Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue.
