IntendedUse:TheEpstein-BarrVirus(EBV)IgG-ELISAisintendedforthequalitativedeterminationofIgGclassantibodiesagainstEpstein-Barrvirus(EBNA)inhumanserumorplasma(citrate).
Introduction:Epstein-BarrVirus(EBV)isamemberoftheherpesvirusfamily(Gammasubgroup,DNAvirusof120-200nm)andoneofthemostcommonhumanviruses.Thevirusoccursworldwide,andmostpeoplebecomeinfectedwithEBVsometimeduringtheirlives.TransmissionofthevirusisalmostimpossIBLetopreventsincemanyhealthypeoplecancarryandspreadthevirusintermittentlyforlife.InfantsbecomesusceptibletoEBVassoonasmaternalantibodyprotectiondisappears.Infectionofchildrenusuallycausesnosymptoms.Infectionduringadolescenceoryoungadulthoodcausesinfectiousmononucleosis35%to50%ofthetime.
Infectiousmononucleosisisalmostneverfatal.TherearenoknownassociationsbetweenactiveEBVinfectionandproblemsduringpregnancy,suchasmiscarriagesorbirthdefects.Althoughthesymptomsofinfectiousmononucleosisusuallyresolvein1or2months,EBVremainsdormantorlatentinafewcellsinthethroatandbloodfortherestofthepersonslife.Periodically,theviruscanreactivateandiscommonlyfoundinthesalivaofinfectedpersons.Thisreactivationusuallyoccurswithoutsymptomsofillness.
EBValsoestablishesalifelongdormantinfectioninsomecellsofthebodysimmunesystem.AlateeventinaveryfewcarriersofthisvirusistheemergenceofBurkitt´slymphomaandnasopharyngealcarcinoma,butEBVisprobablynotthesolecauseofthesemalignancies.

SEROlogy:monospottest,DetectionofantibodiesbyELISA
TheoptimalcombinationofserologictestingconsistsofthetitrationoffourMarkers:IgMandIgGtotheviralcapsidantigen(VCA),IgMtotheearlyantigen,andantibodytoEBVnuclearantigen(EBNA).IgMtoVCAappearsearlyininfectionanddisappearswithin4to12weeks.IgGtoVCAappearsintheacutephase,peaksat2to4weeksafteronset,declinesslightly,andthenpersistsforlife.
Ifantibodiestotheviralcapsidantigenarenotdetected,thepatientissusceptibletoEBVinfection.
Principlesoftheassay:ThequalitativeimmunoenzymaticdeterminationofIgG-classantibodiesagainstEpstein-BarrVirusisbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique.
MicrotiterstripwellsareprecoatedwithrecombinantEpstein-BarrVirusnulcearantigenEBNA-1tobindcorrespondingantibodiesofthespecimen.AfterwashingthewellstoremoveallunboundsamplematerialhorserADIshperoxidase(HRP)labelledanti-humanIgGconjugateisadded.ThisconjugatebindstothecapturedEpstein-BarrVirus-specificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine-(TMB)substratewhichgivesabluereactionproduct.TheintensityofthisproductisproportionaltotheamountofEpstein-BarrVirus-specificIgGantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader.
StorageandStABIlity:Thereagentsarestableuptotheexpirydatestatedonthelabelwhenstoredat2...8°C.
LimitationsoftheTest:Anegativeresult(IgGorIgM)cannotexcludeaninfectionwithEpsteinBarrVirus.Especiallyintheearlystateofinfectionitispossiblethattheamountofantibodiesisbelowthedetectionlimit.IncaseofclinicalsUSPiciontoEBVorequivocaltestresultitisrecommendedtotestanothersampleafter2-3weeks.
Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasserologicaldata.
Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue.
