Introduction | Chlamydiaearenonmotile,Gramnegativeandobligatoryintracellulargrowingbacteriawhichformcharacteristicinclusionswithinthecytoplasmofparasitizedcells.TheyareeasilyvisIBLeinthelightmicroscope.ThreedifferentChlamydiaspeciespathogenicforhumansareknown:Chlamydiatrachomatis,ChlamydiapneumoniaeandChlamydiapsittaci,andonespeciesonlypathogenicforanimals(C.pecorum).Chlamydiatrachomatisisthemostprevalentagentofsexuallytransmitteddiseasesworldwide(400-500millioncases)andthenumberofinfectionsisconstantlygrowing,duringchildbirth,causingconjunctivitisorpneumoniainnewborns.Untreatedcasesofchlamydialinfectioncanleadtochronicsalpingitis,possiblyresultinginectopicpregnancyorinfertility.Inmales,C.trachomatisisamajorcauseofnon-gonococcalurethritis. AsevereprobleminChlamydiainfectionsisthefrequentasymptomaticinsidiouscoursewhichmayresultintheinitiationofchronicdiseases.Inmanyinstancesprimaryinfectionsarenotrecognizedandonlythesequelaecausedbyascended,persistingagentsarediagnosed. Principlesoftheassay | ThequalitativeimmunoenzymaticdeterminationofIgG-classantibodiesagainstChlamydiatrachomatisisbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique. MicrotiterstripwellsareprecoatedwithChlamydiatrachomatisantigenstobindcorrespondingantibodiesofthespecimen.AfterwashingthewellstoremoveallunboundsamplematerialhorserADIshperoxidase(HRP)labelledanti-humanIgGconjugateisadded.ThisconjugatebindstothecapturedChlamydia-specificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct. TheintensityofthisproductisproportionaltotheamountofChlamydia-specificIgGantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader. |

LimitationsoftheTest | Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasserologicaldata. Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue. TheChlamydiatrachomatisantigenwhichiscoatedontheplatesiscomprisedofelementarybodies.AcrossreactionwithChlamydiapneumoniaecannotbeexcludedwithseracontainingantibodiestoLPSandMOMP. |
