Introduction | HerpessimplexisanenvelopedDNAvirus(150-200nmindiameter)belongingtothealpha-herpesviridae.Basedonantigenic,biochemicalandBIOLOGicallydifferencesitcanbedividedintotwoSEROtypes,HSV-1andHSV-2.Manistheonlyknownnaturalhostandsourceofthevirus.HSVType1typicallycausesoralherpes,whileHSV-type2typicallyaffectsthegenitalarea.Mostofthetime,HSV-1andHSV-2areinactive,orsilent,andcausenosymptoms,butsomeinfectedpeoplehaveoutbreaksofblistersandulcers.OnceinfectedwithHSV,peopleremaininfectedforlife.Herpessimplexvirusesareamongstthemostcommoninfectiousagentsofman,andeitherHSVtypeappearstobecapableofinfectingsimilarbodysites.Ahighpercentageoftheadultpopulationisseropositive(appr.90%HSV-1,independenceonthesocioeconomicstatus10-30%HSV-2).PrimaryHSV-1infectionusuallyoccursinearlychildhood(6to18monthsofage).HSV-2usuallyproducesmildsymptoms,andmostpeoplehavenorecognizedsymptoms.PersonsatriskarechildrenwithinheritedT-celldeficienciesandpatientswhoareimmunosuppressedbecauseofinfection(e.g.HIV),transplantation,orcancertherapy. Principlesoftheassay | ThequalitativeimmunoenzymaticdeterminationofIgG-classantibodiesagainstHSVType1+2isbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique.MicrotiterstripwellsareprecoatedwithHSVType1+2antigenstobindcorrespondingantibodiesofthespecimen.AfterwashingthewellstoremoveallunboundsamplematerialhorserADIshperoxidase(HRP)labelledanti-humanIgGconjugateisadded.ThisconjugatebindstothecapturedHSVType1+2-specificantibodies.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct.TheintensityofthisproductisproportionaltotheamountofHSVType1+2-specificIgGantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolor.Absorbanceat450nmisreadusinganELISAmicrowellplatereader. |

LimitationsoftheTest | Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.ThepresenceofIgGantibodiestoHSV1+2donotimplyaprotectionforafurtherHSV1+2infection.CrossreactivitywithVZVcannotbeexcluded.AsignificanthighHSVantibodytitercanbecausedbyaVZVinfectionandviceversa.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasserologicaldata.Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue. |
