Introduction:HelicobacterpyloriisaspiralbacteriumculturedfromhumangastricmucosadiscoveredbyB.J.Marshallin1982.StudieshaveindicatedthatthepresenceofH.pyloriisassociatedwithavarietyofgastrointestinaldiseasesincludinggastritis,duodenalandgastriculcers,non-ulcerdyspepsia,andgastricadenocarcinomaandlymphoma.Theorganismispresentin95-98%ofpatientswithduodenalulcersand60-90%ofpatientswithgastriculcers.
Allofthetestingperformedonbiopsysamplesissubjecttoerrorsrelatedtosamplingandinterferenceofcontaminatedbacteria.AnELISAtestforthepresenceofH.pylorispecificIgMantibodyisthetechniqueofchoiceforserologictestsbecauseofitsaccuracyandsimplicity.
Principleofthetest:PurifiedH.pyloriantigeniscoatedonthesurfaceofmicrowells.Dilutedserumisaddedtothewells,andtheH.pyloriIgMspecificantibody,ifpresent,bindstotheantigen.Allunboundmaterialsarewashedaway.Enzymeconjugateisadded,whichbindstotheantibody-antigencomplex.ExcessenzymeconjugateiswashedoffandasolutionofTMBReagentisadded.Theenzymeconjugatecatalyticreactionisstoppedataspecifictime.TheintensityofthecolorgeneratedisproportionaltotheamountofIgM-specificantibodyinthesample.Theresultsarereadbyamicrowellreadercomparedinaparallelmannerwithcalibratorandcontrols.
