2.GeneralInformation Eachyearduringthewintermonths,RSvirusesspreadheavilyamongstchildrenandinfants.There arerecurringinfectionbyRSviruseseveryyear.Voluntarystudieswithadultshavedemonstrated thatareinfectionwithRSvirusesispossible.Theincubationtimeis2-6days.RSvirusesare paramyxoviruseswithadiameterof90-140 nm.ThemostnoticeableconnectionofRSV infectionswithrespiratoryinfectionsandspecificsyndromeswasdetectedininfantsupto6months ofagewithbronchiolitisorpneumonia.Inolderinfantsorsmallchildrenthediseaseismilder.In 25%ofinfectionsoftherespiratorytractRSVinfectionsaredetectable.AsreinfectionswithRSV arepossible,itisassumedthatthesereinfectiousantibodiesareresponsibleforthemildcourseof thediseaseinadults,beingsimilartoacold. However,especiallyintheearlyyears,serum antibodiesarenoeffectiveprotectionagainstinfectionsoftherespiratorytract.Therefore,this pathogenmaycausebronchiolitisor,ininfantsupto4monthsofage,pneumonias.Basedontheir antigenrelationship,RSVisolatesaredifferenciatedintotwomajorgroups(AandB).Thesurface glycoproteinsofthevirus(Gglycoproteinandfusionglycoprotein)causetheproductionofvirusneutralizingantibodies.ObviouslytheGglycoproteinsofgroupsAandBareverydifferent,while theFglycoproteinsofbothgroupsshowahighantigenconcurrence.Thecomplementbinding reactionisunsatisfactoryfortheSEROlogicaldiagnosisofRSV.Enzymeimmunoassaysareof diagnosticvaluefortheserologicaldiagnosisofRSVinfections,astheyareverysensitiveand allowthedifferentiationofantigensintothevariousimmunoglobulinclasses.InRSVinfections,it ispossiblethattheIgMantibodyresponseismissingorsoweakthatareliableinterpretationofthe resultsisimpossible.ThedetectionofIgGantibodiesinasinglesampleisnoevidenceforanacute infectionas,insomespecimenIgA,antibodiesmaypersistmonthsandyears.Themethod recommendedforserologicaltestingofacuteRSVinfectionsisthedeterminationofIgGantibodies inserumpairswithsignificanttiterrise.
3.PrincipleoftheTest TheGenWayRSVIgGantibodytestkitisbasedontheprincipleoftheenzymeimmunoassay (EIA).RSVantigenisboundonthesurfaceofthemicrotiterstrips.Dilutedsampleserumorreadyto-usestandardsarePipettedintothewellsofthemicrotiterplate.AbindingbetweentheIgG antibodiesoftheserumandtheimmobilizedRSVantigentakesplace.Afteraonehourincubation atroomtemperature,theplateisrinsedwithdilutedwashsolution,inordertoremoveunbound material.Thenready-to-useanti-human-IgGperoxidaseconjugateisaddedandincubatedfor30 minutes.Afterafurtherwashingstep,thesubstrate(TMB)solutionispipettedandincubatedfor20 minutes,inducingthedevelopmentofabluedyeinthewells.Thecolordevelopmentisterminated bytheadditionofastopsolution,whichchangesthecolorfrombluetoyellow.Theresultingdyeis measuredspectrophotometricallyatthewavelengthof450nm.TheconcentrationoftheIgG antibodiesisdirectlyproportionaltotheintensityofthecolor
