FurtherapplicationsinotherbodyfluidsarepossIBLeandcanberequestedfromtheTechnical ServiceofGenWay. Thisassayisintendedforresearchuseonly.
2.GeneralInformation Infectiousmononucleosisisanacutelymphoproliferativediseasethatiscommoninchildrenand youngadultsandiscausedbytheEpstein-BarrVirus.TheEBVisoneoftheherpesviruses4 (gamma). Characteristicclinicalfeaturesinclude:1.Fever,sorethroat,andlymhadenopathy;2.anassociated absolutelymphocytosisgreaterthan50%ofwhichatleast10%areatypicallymphocytesinthe peripheralblood;3.developmentoftransientheterophilandpersistentantibodyresponsesagainst EBV;and4.abnormalliverfunctiontests. In4%ofinfectedyoungadultsthereisanictericmanifestationandin50%splenomegaly.Aninfectiousmononucleosissimilarsyndromecanbecausedbycytomegalovirus,toxoplasmosisandother viralinfections;differentialdiagnosisdependsonthelaboratoryresults,EBVbeingtheonlyvirus, thatstimulatestheproductionofheterophilantibodies.EBVispresentinsalivafromindividuals withacuteinfectionsmononucleosis,andexcretionofthevirusfromtheoropharynxcontinuesfor manymonthsafterclinicaldisease,thisbeingoneoftheprinciplewaysoftransmissionofthe disease. Essentialtothediagnosisisthedocumentationofanincreaseintherelativeandabsolutenumberof lymphocytesandatypicallymphocytes.Duringdisease,cellsofthelymphocyticseriesmayconstitute50%to60%ofperipheralbloodleucocytes,andatypicallymphocytesusuallyrepresentatleast 10%ofthem.Alterationinsomeofthetestsforhepaticfunctionandahightiterofheterophilantibodiesareobserved. SEROlogicaltestslikeEIAareveryusefulforthedetectionofanti-EBVIgGandIgMantibodies, especiallyincases,whereheterophilantibodiesareabsent.DevelopmentofVCAIgGantibodies occursearlyindisease,reachespeaktiterswithin2to4weeksandpersistsformanyyears, probablyforlife.VCAIgMantibodies,however, occuratthebeginningbutdecreaseoreven disappearwithinafewmonths.
3.PrincipleoftheTest TheGenWayEBVVCAIgAantibodytestkitisbasedontheprincipleoftheenzymeimmunoassay (EIA).EBVVCAantigenisboundonthesurfaceofthemicrotiterstrips.Dilutedsampleserumor ready-to-usestandardsarePipettedintothewellsofthemicrotiterplate.AbindingbetweentheIgA antibodiesoftheserumandtheimmobilized EBVVCAantigentakesplace.Afteraonehour incubationatroomtemperature,theplateisrinsedwithdilutedwashsolution,inordertoremove unboundmaterial.Thenready-to-useanti-human-IgAperoxidaseconjugateisaddedandincubated for30minutes.Afterafurtherwashingstep,thesubstrate(TMB)solutionispipettedandincubated for20minutes,inducingthedevelopmentofabluedyeinthewells.Thecolordevelopmentis terminatedbytheadditionofastopsolution,whichchangesthecolorfrombluetoyellow.The resultingdyeismeasuredspectrophotometricallyatthewavelengthof450nm.Theconcentration oftheIgAantibodiesisdirectlyproportionaltotheintensityofthecolor.
