ShareonEmailFacebookTwitterLinkedInRedditWechatNeuromyelitisoptica(NMO)isanautoimmunediseasewithahighrateofblindnessandpositiveforthedetectionofaquaporin‐4antibody(AQP4)inmostpatients.NMOacuteattacksaremanagedbyhigh‐dosesofintravenousmethylprednisolonefollowedbyoraltaper,andifsymptomsfailtoresolve,therapeuticplasmaexchange(TPE)isadded.TPEcanremovepathologicalantibodiesandinflammatoryfactorsleadingtoclinicalimprovement.
Atotalof40TPEfluidcollectionsfromthefirsttofifthTPEtreatmentswereobtainedfromeightpatients.Exosomeswereisolatedbyultracentrifugation.MassspectrometryanalyseswereusedtocompareproteinchangeinTPEfluidcollectionexosomesafterthefirsttothefifthTPEtreatmentsinthesepatients.
Wedetected647exosomeproteinsthroughdata‐independentacquisitionanalysis.ItwasfoundthatsomeunknownfunctionalantibodyfragmentsandcomplementpathwayproteinsdecreasedafterTPEtreatment.Theresultsrevealedasignificantinvolvementofthefollowingtwokeypathways:theprimaryimmunodeficiencyandsystemiclupuserythematosusthatmaybeassociatedwithNMOpathophysiologyandTPEtreatmentefficacy(P .05).AseriesofcomplementproteinsmaycontributetoNMO;inaddition,thefollowingproteinsincreasedwithplasmaexchange:complementfactorH‐relatedprotein5,bridgingintegrator2,neuroplastin,pigmentepithelium‐derivedfactor,ficolin‐1,extracellularmatrixprotein1,andfattyacid‐bindingprotein5.
OurstudymayprovideanewperspectiveonthepathogenesisandtreatmentefficacyofNMO.
