IntendedUse:TheChagas(Trypanosomacruzi)IgG-ELISAisintendedforthequalitativedeterminationofIgGclassantibodiesagainstTrypanosomacruziinhumanserumorplasma(citrate).
Introduction:ChagasdiseaseoccursinMiddleandSouthAmerica.Itisaninfectiousdiseasetransmittedtohumansthroughthebitewoundcausedbyaninfectedbugofthefamilyreduviidae.TheinfectioniscausedbyTrypanosomacruziaunicellularparasite.Theinfectionpassesthroughdifferentphasesandcausesanoftenchronicdisease.4to5millionpeopleinSouthAmericaaresufferingfromit.Peoplelivingunderpoorconditionsaremainlyjeopardized.Upto10%ofallinfectionsarefatal;bABIesandneonateshaveaspecialrisk.
Theinfectionistransmittedtohumansbydifferentbugspeciesofthefamilyreduviidae.Theparasitedoesnotenterthehumanbodywiththebitofthebug.Trypanosomacruzilivesinfaecesofthebugandinvadesintohumansbyskinwounds.Newbornsareatriskbyinfectioninutero.AninfectionviabloodtransfusionisalsopossIBLe.
1-2%ofallinfectedpeopleshowsymptomsafteranincubationtimeofonetofourweeks.Childrenupto15yearsoldaremainlyaffected.Thediseasepassesthroughthreedifferentphases.
Duringacutephasefever,diarrhoea,gripes,swollenlymphnodesandswellingofthewholebodyappears.Especiallyinneonatesandinfantsinflammationofheartorbrainarepossible.Theacutephaselastsforca.4weeks.TheacuteChagasdiseaseismostlyadiseaseofchildren.Inmanycasesthediseaseishealedup.
Thefollowinglatencyperiodischaracterizedbylackofsymptomsformostofthepatients.Occasionallyaweakeningoftheimmunesystemtakesplace.Thisphasecanlastseveralyears.
10to20%ofinfectedpeoplereachthechronicphase.Differentinnerorganslikeheart,intestinaltractorneuronalsystemcanbeaffected.Patientsoftendiebesuddencardiacdeathorastheresultofachronicheartinsufficiency.Theprognosisdependsuponthedegreeofheartvariance.
Avarietyofdiagnosticmethodshavebeenused,butdetectionofantibodiestoT.cruziantigensremainsthestrongestmethodtodiagnoseinfection.
Principlesoftheassay:ThequalitativeimmunoenzymaticdeterminationofIgG-classantibodiesagainstChagas(Trypanosomacruzi)isbasedontheELISA(Enzyme-linkedImmunosorbentAssay)technique.
MicrotiterstripwellsareprecoatedwithTrypanosomacruziantigenstobindcorrespondingantibodiesofthespecimen.AfterwashingthewellstoremoveallunboundsamplematerialhorserADIshperoxidase(HRP)labelledProteinAconjugateisadded.Thisconjugatebindstoantigen-antibodycomplexes.TheimmunecomplexformedbytheboundconjugateisvisualizedbyaddingTetramethylbenzidine(TMB)substratewhichgivesabluereactionproduct.
TheintensityofthisproductisproportionaltotheamountofTrypanosomacruzi-specificIgGantibodiesinthespecimen.Sulphuricacidisaddedtostopthereaction.Thisproducesayellowendpointcolour.Absorbanceat450nmisreadusinganELISAmicrowellplatereader.
StorageandStability:Thereagentsarestableuptotheexpirydatestatedonthelabelwhenstoredat2...8°C.
LimitationsoftheTest:Bacterialcontaminationorrepeatedfreeze-thawcyclesofthespecimenmayaffecttheabsorbancevalues.Diagnosisofaninfectiousdiseaseshouldnotbeestablishedonthebasisofasingletestresult.Aprecisediagnosisshouldtakeintoconsiderationclinicalhistory,symptomatologyaswellasSEROlogicaldata.
Inimmunocompromisedpatientsandnewbornsserologicaldataonlyhaverestrictedvalue.
CrossreactionoftheantigenswithantibodiesagainstLeishmania,TrypanosomabruceirhodesienseandTrypanosomabruceigambiensecannotbeexcluded.
