2.GeneralInformation Eachyearduringthewintermonths,RSvirusesspreadheavilyamongstchildrenandinfants.There arerecurringinfectionbyRSviruseseveryyear.Voluntarystudieswithadultshavedemonstrated thatareinfectionwithRSvirusesispossible.Theincubationtimeis2-6days.RSvirusesare paramyxoviruseswithadiameterof90-140 nm.ThemostnoticeableconnectionofRSV infectionswithrespiratoryinfectionsandspecificclinicalsyndromeswasdetectedininfantsupto6 monthsofagewithbronchiolitisorpneumonia.Inolderinfantsorsmallchildrenthediseaseis milder.In25%ofinfectionsoftherespiratorytractRSVinfectionsaredetectable.Asreinfections withRSVarepossible,itisassumedthatthesereinfectiousantibodiesareresponsibleforthemild courseofthediseaseinadults,beingsimilarto acold.However,especiallyintheearlyyears, serumantibodiesarenoeffectiveprotectionagainstinfectionsoftherespiratorytract.Therefore, thispathogenmaycausebronchiolitisor,ininfantsupto4monthsofage,pneumonias.Basedon theirantigenrelationship,RSVisolatesaredifferenciatedintotwomajorgroups(AandB).The surfaceglycoproteinsofthevirus(Gglycoproteinandfusionglycoprotein)causetheproductionof virus-neutralizingantibodies.ObviouslytheGglycoproteinsofgroupsAandBareverydifferent, whiletheFglycoproteinsof bothgroupsshowahighantigenconcurrence.Thecomplement bindingreactionisunsatisfactoryfortheSEROlogicaldiagnosisofRSV.Enzymeimmunoassaysare ofdiagnosticvaluefortheserologicaldiagnosisofRSVinfections,astheyareverysensitiveand allowthedifferentiationofantigensintothevariousimmunoglobulinclasses.InRSVinfections,it ispossiblethattheIgMantibodyresponseismissingorsoweakthatareliableinterpretationofthe resultsisimpossible.ThedetectionofIgGantibodiesinasinglesampleisnoevidenceforanacute infectionas,insomesampleIgA,antibodiesmaypersistmonthsandyears.Themethod recommendedforserologicaltestingofacuteRSVinfectionsisthedeterminationofIgGantibodies inserumpairswithsignificanttiterrise.
3.PrincipleoftheTest TheGenWayRSVIgAantibodytestkitisbasedontheprincipleoftheenzymeimmunoassay (EIA).RSVantigenisboundonthesurfaceofthemicrotiterstrips.Dilutedsampleserumorreadyto-usestandardsarePipettedintothewellsofthemicrotiterplate.AbindingbetweentheIgA antibodiesoftheserumandtheimmobilizedRSVantigentakesplace.Afteraonehourincubation atroomtemperature,theplateisrinsedwithdilutedwashsolution,inordertoremoveunbound material.Thenready-to-useanti-human-IgAperoxidaseconjugateisaddedandincubatedfor30 minutes.Afterafurtherwashingstep,thesubstrate(TMB)solutionispipettedandincubatedfor20 minutes,inducingthedevelopmentofabluedyeinthewells.Thecolordevelopmentisterminated bytheadditionofastopsolution,whichchangesthecolorfrombluetoyellow.Theresultingdyeis measuredspectrophotometricallyatthewavelengthof450nm.TheconcentrationoftheIgA antibodiesisdirectlyproportionaltotheintensityofthecolor.
