2.GeneralInformation Helicobacterpylori,a2.5-3µmlong,twistedorhelicalgram-negativegermisresponsiblefor80-90%ofB-gastritiscasesandissUSPectedtobeamajorcofactorforthedevelopmentofgastricand duodenalulcers.Uptonow,Helicobacterpyloricouldbemostreliablydetectedbyculturingfrom mucousmembranebiopsies;alsoaureasetest wasemployedforidentification.Thesedetection methodsareonlysuccessfulforarelativelyhighgermcountfromtheinoculateandrequirean identificationdirectlyaftertakingthebiopsy.Recentlyitwasshownthatadetectionof Helicobacterpyloribyindirectimmunofluorescence(IIF)shouldbesuperiortotheabovemethods. TheidentificationbyIIFisalsoadvantageousasitisnotnecessarytoperformthedetermination directlyafterthebiopsy;thuspreparationscanbesenttospecializedlaboratories.Thecolonisation ofthegastricandduodenalmucousmembranes byHelicobacterpyloricanalsobedetected SEROlogicallybytheperformanceofanenzymeimmunoassay(ELISA)orbyWesternBlot. IndividualswithconfirmedexpositiontoHelicobacterpylorioftenshowapositiveserological result.SinceantibodiespersistforalingertimeafteraHelicobacterpyloriinfection,seropositive individualsarealsofoundamongsymptom-freeindividuals.Theratioofseropositivevaluesrises withage.BythedeterminationofimmunoglobulinsinELISAandbythedetectionofIgG,IgAand IgMantibodiesagainstspecificproteinsofHelicobacterpyloriintheWesternBlot,itshouldbe possibletodiagnoseanacuteinfectionwithHelicobacterpylori,evenifnogermscanbefound. ThedetectionofHelicobacterpyloriisofremarkableinterestforthetherapy.Bismuthsaltsand antibioticsusetokillthegerms,andtheseshouldcomplementorreplacetheusualtherapywith antacidicandH2blockingagents.Serologicalmonitoringcouldthusalsobeemployedtocontrol thesuccessofanti-microbialtherapy.
3.PrincipleoftheTest TheGENWAYHelicobacterpyloriIgGantibodytestkitisbasedontheprincipleoftheenzyme immunoassay(EIA).Helicobacterantigenisboundonthesurfaceofthemicrotiterstrips.Diluted sampleserumorready-to-usestandardsarePipettedintothewellsofthemicrotiterplate.Abinding betweentheIgGantibodiesoftheserumandtheimmobilizedHelicobacterantigentakesplace. Afteraonehourincubationatroomtemperature,theplateisrinsedwithdilutedwashsolution,in ordertoremoveunboundmaterial.Thenready-to-useanti-human-IgGperoxidaseconjugateis addedandincubatedfor30minutes.Afterafurtherwashingstep,thesubstrate(TMB)solutionis pipettedandincubatedfor20minutes,inducingthedevelopmentofabluedyeinthewells.The colordevelopmentisterminatedbytheadditionofastopsolution,whichchangesthecolorfrom bluetoyellow.Theresultingdyeismeasuredspectrophotometricallyatthewavelengthof450nm. TheconcentrationoftheIgGantibodiesisdirectlyproportionaltotheintensityofthecolor.
